Ticket Parade Application Form – Dance

This video chat series has now been condensed to a single session.

Video chat date: 9.00am – 10.30am, Saturday 15 August
Dance applications close: Wednesday 12 August, 5pm
Applicants: Students in Years 9 – 12 (studying, or committed to Dance) 
Cost: Free 

Important Information 
Before you apply, please note the following and discuss with your parent/carer.  

  1. We will contact you and your parent/carer by Thursday 13 August, to confirm your registration.
  2. This program will take place over video chat. Each session will be supervised by C-A-C Art Educators, and all facilitators involved have valid NSW Working with Children clearance checks.

Online Participation
I give permission for my child to participate in online video chats, coordinated and supervised by Campbelltown Arts Centre (part of Campbelltown City Council). 

Safety Information:

All educators and arts workers in the program have valid NSW Working With Children Checks.

C-A-C (Campbelltown Arts Centre) understands student participants, from different schools, may go on to connect outside of the program. We encourage students and parents/carers to practice safe and respectful online activity. 

We will advise students on safe and respectful online behaviour during week 1 video session, and we will co-develop some group rules/ expectations with the students.

Permission to Campbelltown City Council c/- Campbelltown Arts Centre to use the image and likeness of my child as outlined below:

• Recordings and images from this program may be edited and used to publicise and promote Campbelltown Arts Centre, or as part of reporting requirements for funding. 

• Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of participants (including Your Child) for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the Campbelltown City Council and Campbelltown Arts Centre website.

• This permission applies to workshops, performances and other activities conducted by C-A-C. I acknowledge that if at any stage I no longer wish for My Child’s images to be used, I will notify the Arts Centre in writing via email to [email protected]
