Please provide the following information in regards to students who will access / view the video.

(1)  I acknowledge responsibility for, and commit to the following requirements for the workshop:

A staff member, with experience in Dance, PE and/or Aboriginal education will lead the workshop. C-A-C is providing the video as a learning aid, and requires in-person facilitation from

educators with appropriate skills in dance education and cultural awareness.

This video is not to be shared or broadcast to other parties without prior consent. Any copies of the video must be deleted after use in term 4. This is to respect artists’ rights and ensure correct use of resource.

Dance workshop requires a hall, gym or performing arts room, appropriate floors & no furniture. We recommend video be presented on a large data projector to reference during the workshop.

Students should bring a water bottle and wear comfortable clothes for dancing and physical exercise

Students and relevant teachers should be briefed on the program and aware of their participation ahead of the day

(2) Photo Permission*

We request you provide photo or video documentation of your workshop(s) to help us in reviewing and reporting on the project.

Campbelltown City Council c/- Campbelltown Arts Centre seeks permission to use the photographs and/or video of your group, provided by the school, to be used as outlined below: 

-Recordings and images from this program may be edited and used to publicise and promote Campbelltown Arts Centre, or as part of reporting requirements for funding.

-Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video of your group for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the Campbelltown City Council and Campbelltown Arts Centre website, partners’ websites and on social media outlets including Facebook and Instagram.

-If at any stage the group or individuals no longer wish for these images to be used, they can notify the Arts Centre in writing via email to [email protected]
